The 4149A104 tank is the Champion family tank, part of the Champion 4 HET 1.28 gpf system, so it mates with the Champion 4 bowl it was built for. Tanks are not interchangeable across toilet families, so if the bowl in the room is not a Champion, this is the wrong tank. Check the bowl's model number before ordering a replacement tank on its own.

A chrome finish trip lever is included with the tank, so there is nothing extra to specify for the handle. What is not included is the bowl, the tank-to-bowl gasket and bolt set, or a supply line, so a plumber pulling a cracked tank should have a fresh tank-to-bowl kit and a new supply on the truck.
