1) Does the Eaton LA2400PRF breaker come with lugs, or do I need to order terminals separately?
Terminals ship separately from the breaker on the LA frame, so the copper line and load terminals have to be ordered as their own line item. Plan for that when you price out the job, otherwise the breaker lands on site with nowhere to land the conductors.
2) Is the LA2400PRF a 400A fixed trip, or can the trip setting be adjusted down?

The LA frame is rated 600 Vac and 250 Vdc, so 480Y/277 and 600V ac systems are within rating, as is 250V dc. It is a two-pole device, so on a three-phase system it serves two-pole loads, not a full three-phase feeder.
