No. The PDF-161 is a cover plate, nothing more, and it does not restore the rating of a fire door that has been bored through. A hole in a fire door has to be filled by a method the door's listing allows. Use the filler plate on non-rated openings, or where the hole is cosmetic only.
2) What is a hole filler plate actually for, and do I need one on both sides of the door?
A hole filler plate is a flat steel disc that caps an abandoned bore hole in a door, usually left when a lockset is removed, moved, or swapped for hardware with a different prep. It is cover, not structure, so it does not restore the door's fire rating or add security. Doors are bored all the way through, so the hole shows on both faces. Plan on two PDF-161 plates per opening if both sides are visible.
3) Is 16-gauge steel heavy enough for a filler plate in a school or hospital corridor?
The PDF-161 is 16 gauge, .060 inch steel, which is the same weight of material used for commercial wrap arounds and armor plate. It will not oil can or dent from a cart strike the way a thin decorative cover would. That is the reason it is specified for commercial rather than residential use.

A filler plate like the PDF-161 simply caps the 2-5/8 inch area over an abandoned bore and leaves the door face otherwise as-is, which is what you want when nothing is going back in that spot. A wrap around plate is for when hardware is staying in the opening but the door face around it is chewed up or the prep needs reinforcing, since it folds over the edge of the door and covers a much larger area. If the hole is being permanently retired, the filler plate is the cleaner and smaller repair.
