1) Will the Don-Jo SB-12 slide bolt work on a door with no frame prep, or do I have to mortise the jamb?
Either way works. The SB-12 comes with both a rim strike and a mortise strike, so you can surface-mount the strike on the frame face when you don't want to cut the jamb, or mortise it in when you want the flush look. The bolt body itself is surface applied to the door, so the door needs no prep beyond screw holes.
2) Is the Don-Jo SB-12 a flush bolt or a surface bolt? I need to secure the inactive leaf of a pair of doors.
It is a surface-applied slide bolt, not a flush bolt mortised into the door edge. It screws onto the face of the inactive leaf and throws into a strike at the head or the threshold. If your spec calls for hardware concealed in the door edge, this is the wrong style and you want a true flush bolt instead.
3) Does the Don-Jo SB-12 surface bolt meet an ANSI spec I can put on a submittal?
It conforms to ANSI L04151, which is the ANSI/BHMA function designation for a surface bolt. That is the number to cite on a hardware schedule or submittal when the spec calls out a surface bolt by function rather than by manufacturer.
4) Can I use a Don-Jo SB-12 slide bolt on a fire rated door?
