It comes down to current. The 2-wire version uses 20 gauge conductors and carries 4.0 continuous amps at 24VDC with a 16.0 amp surge per pair for 0.5 seconds, so it is the one for an electrified lockset or ELR exit device that draws real current, or where you want headroom for inrush. The 4-wire version is 26 gauge and rated 1.0 continuous amp at 24VDC with no surge rating, so use it when you need two separate low-current circuits, such as power plus a signal, rather than one high-current run.

Same hinge, one extra function. The ETM-1 adds a built-in SPDT monitor switch for door position status reporting back to an access control panel or alarm point, while the ETH-1 only passes power.
