Yes. The TP720 raised surface cover is sized for 4-11/16 inch square boxes, which is the larger of the two common commercial square box sizes (the other being 4 inch). It will not sit on a 4 inch square box, and it is not a flat cover: the raised configuration adds 0.25 inch of depth and 9.0 cubic inches of usable volume above the box, which counts toward your box fill.
2) Which Crouse-Hinds TP cover do I order for a duplex receptacle instead of a switch?
For one duplex receptacle, order the TP722. The toggle switch version is the TP720. Both are the same 4-11/16 inch raised steel cover; only the device cutout differs, so the box and mounting are identical and you are choosing purely on what device goes in the opening.
3) Can I get a 4-11/16 inch raised cover for two devices on one box?
Yes, two-device versions exist in this Crouse-Hinds raised cover line: TP726 for two toggle switches, TP728 for two duplex receptacles, and TP741 for two GFI receptacles.
