2) Do these ground pigtails come with a screw or a ring terminal attached?
No. The PT40B is plain #12 solid bare copper cut to 7 inches, with no green grounding screw, clip, or crimped ring terminal on either end. If you want a pigtail with the grounding screw already fitted, or a ground clip for the box, that is a separate item you need to order alongside these.
3) Is 7 inches long enough for a ground pigtail in a standard device box?
For a typical single gang box, yes. The PT40B gives 7 inches of #12 copper, which leaves enough free conductor to reach out past the box face for the splice and still fold back behind the device once the receptacle is set. Deep boxes stacked several gangs wide or pigtails that have to travel across a large junction box are where a 7 inch leg gets tight.
4) Is the PT40B insulated or bare wire?

Yes. Solid #12 copper is within the range of common twist-on wire nuts and push-in lever or stab connectors rated for #12 solid, and solid wire is easier to seat in a push-in connector than stranded. Strip the pigtail back to the connector manufacturer's strip gauge before inserting.
