If you need a true vertical riser out of the deck, a 45 degree stubby is the wrong geometry for it.

Yes. The inside throat is enlarged specifically so PVC female adapters install easily where the job calls for one.
5) Is the stubby left in the slab or removed after stripping forms?
It comes out. The fitting has a pullout tab for removal after the concrete is poured, and the 'T' slot nail hole releases the nail head when forms are removed. What stays in the slab is the ENT raceway and its stubdown.
6) Is the stubby tough enough to survive the concrete pour?
It is molded from PE (polyethylene), chosen to stand up to the pour while still flexing enough to be pulled back out with the tab once the forms come off. The body is black for identification on the deck.
