2) How much head frame and top rail do I need to use an LCN 4020 drop plate?
Minimum 1-3/4 inch (44 mm) head frame and 2-3/4 inch (70 mm) top rail. Measure the opening before you order: if the frame face is under 1-3/4 inches or the door's top rail is under 2-3/4 inches, the 4020-18G will not mount and you need a different mounting solution.
3) Will the 4020-18G plate work with a closer that isn't an LCN 4020?

ADA compliant, yes. The plate is a commercial-duty accessory and does not compromise the accessible-opening compliance of the 4020 closer it mounts. Note that ADA opening force still depends on how the closer itself is sized and adjusted, not on the plate.
