1/2 inch conduit throughout, with one 3/4 inch option. If your feed is 3/4 inch EMT, use the eccentric on the side, since the bottom knockout is 1/2 inch only.

No separate cover needed. The box support cover supplied with the 52171X-CM14 is the device cover and the stud-mounting bracket in one piece, so the box mounts to the stud and is ready for the device without an added ring.

Yes. The 52171X-CM14 has two cable knockouts at each end, so you can feed and pass through with cable without touching the conduit knockouts on the sides. Sides and bottom are set up for 1/2 inch conduit if you need to mix methods on the same box.
