1) Does the ASLB-3 come with the cover and gasket, or do I order those separately?
Cover and gasket ship with the body. The cover screws are retained deep slotted stainless steel, so they stay in the cover when you back them out and resist rust on an outdoor service run. Nothing else is needed to close the body up.
2) Can I use the ASLB-3 with PVC service entrance conduit?

5) Will a 1 inch LB conduit body mount flat against a wall for a service entrance?
Yes. The ASLB-3 body is built to mount flush to the wall, with the cover facing out so you can open it for wire pulling after the conduit is strapped. The throat is smooth, which matters when you are pulling large service conductors through the 90.
