No. The 30-C1212D kit ships without a fuse. It accepts a 13/32 x 1-1/2 inch midget-style fuse, which you order separately and size to the fixture load.

The connector body accepts 14 AWG up to 2 AWG and is rated for both copper and aluminum conductors, so it works on pole feeds run in aluminum as well as copper fixture leads.

Yes. The SLK disconnect fused kit is Listed to UL 486D for submersibility, which is the listing that covers connectors used in wet, below-grade locations like pole bases, handholes and light bases.
4) Does this fuse holder break open to kill power when servicing a street light fixture?
Yes, that is the point of it. The SLK uses a breakaway fuseholder intended for roadway, street, parking lot and area lighting, so a lineman can pull the halves apart at the pole base to isolate the fixture without cutting conductors. The body is bright orange so it is easy to spot inside the base.
5) Is the 30-C1212D a crimp or set-screw connection, and how many line and load conductors does it take?
The 30-C1212D is the crimp version, wired one line conductor to one load conductor in a dual in-line arrangement. IDEAL also builds the SLK disconnect with set screw terminations if you would rather not carry a crimp tool on the truck.
