Use the 1 toggle / 1 GFI version for a GFCI or any decorator style device, since the opening is the rectangular decorator cutout. The 2 toggle / 1 duplex version has the figure-eight duplex cutout and will not accept a GFCI face.

Yes, that is the point of it. The SP device cover mounts directly to the 4-11/16 square box and carries the devices itself, so on exposed or surface work you do not need a separate ring and finish plate. For flush drywall work you would still want a plaster ring instead.
4) Is this cover suitable for a damp or exposed area?
The cover is pre-galvanized steel, which suits dry indoor exposed work such as mechanical rooms, garages and back-of-house. Pre-galvanized is not a wet-location weatherproof cover; outdoors or in wet locations use a gasketed weatherproof cover and box.
5) What does the SP 3-gang cover look like finished, and will it match my other covers?
Plain silver steel with a pre-galvanized finish, unpainted. It matches the standard galvanized look of other Steel City square boxes and covers, so on a run of exposed conduit it blends with the rest of the rough hardware. If you need a painted or stainless appearance in a finished space, this is not it.
