The box stands 3.5 inches in height, so you need at least that much clear depth below the finished wood floor surface, plus room to route conductors in from the side. Over a joist bay or a raised wood subfloor that is usually fine; over a thin sleeper system or directly over concrete it is not, and you would want a shallower or concrete-rated box instead.
2) Is this Wiremold floor box suitable for a concrete slab pour?
No. This is a wood floor box, built for installation into wood flooring and wood subfloor assemblies. A concrete pour needs a box rated for encasement in concrete with the appropriate leveling and pour-stop provisions. Use the wood box only where you are cutting into a wood floor.
3) How big is the cutout for this Wiremold wood floor box?
The box body measures 4.25 inches wide by 4.25 inches deep by 3.5 inches high, so your rough opening in the wood flooring is sized off that 4.25 inch square footprint. Lay it out between joists before you cut, because there is no adjusting a die-cast body once the hole is in a hardwood floor.
