P x MPT means one press end and one male pipe thread end. The press side slips over 1-1/4 inch carbon steel pipe and is crimped with a press tool, and the threaded side is a 1-1/4 inch male NPT that screws into a female threaded port, valve, or piece of equipment. That is what makes it an adapter: it takes you from pressed pipe to threaded connections without welding or threading the pipe in the field.

Both ends are 1-1/4 inch. The press end takes 1-1/4 inch carbon steel pipe and the male thread is 1-1/4 inch, so there is no size step in this fitting. If you need to change size at the same point, you want a reducing adapter or a separate bushing, not the 25115.

No. The 25115 is a carbon steel press fitting and its press end is sized and sealed for carbon steel pipe, not copper tube or PEX. For copper you need a copper press fitting system; for PEX you need PEX fittings. Mixing systems at the press end will not seal.

Not on the press side. The 25115 presses onto plain-end 1-1/4 inch carbon steel pipe with a press tool and jaw, so the pipe only needs to be cut square, deburred, and marked for insertion depth. The only threading involved is the factory male thread on the other end, which you make up with pipe dope or tape into the mating female port. That is the reason crews use these on retrofit work in occupied buildings, where a torch or a threading machine is a problem.

The fitting itself measures 3.642 inches long and 2.205 inches in both height and width, so plan clearance from those numbers plus the swing of your jaw. In tight chases and against joists the jaw, not the fitting, is what runs out of room, so measure from the press end outward before you commit to the layout.

Smart Connect is Viega's unpressed-connection feature: an unpressed joint leaks visibly during pressure testing so a missed press shows up on the test instead of after the walls close. On a large hydronic or compressed air run with dozens of joints, that is the difference between finding one missed crimp at test and chasing it later.
