1) Is this a urinal flush valve or a water closet valve?
Urinal only. The Ultima 6145013 flushes at 0.125 gpf (0.5 Lpf), which is a pint-per-flush urinal rate, far too little water to clear a toilet bowl. For a water closet you need a 1.28 or 1.6 gpf closet valve instead.
2) Is this handle ADA compliant for a public restroom?
The handle is an ADA-compliant non-hold-open type that shuts off automatically after each flush, so a user doesn't have to hold it down. That's the handle characteristic facilities buyers are checking for on public and institutional restrooms.
3) Does the supply have to come in from a particular side?

The diaphragm uses American Standard's EvoLast material, made to resist chlorine and chloramine so it doesn't deteriorate and fail early. The valve body is a proprietary brass alloy that resists dezincification, which is the other common failure mode on aggressive water. For a property manager replacing diaphragms every season, those are the two parts doing the failing.
6) What stops a low-flow urinal valve like this from running on?
Run-on usually comes from debris in the refill orifice holding the diaphragm off its seat. The Ultima uses DynaClean, an integral wiper spring that cleans the refill orifice on every flush to keep that from happening. A high back pressure vacuum breaker is also built in to help prevent dripping at the outlet.
