No valve is included with the TS100EV wall spout. You are buying the spout only, so the tub/shower valve body and rough-in have to be ordered separately. Plan the valve selection around a 1/2 inch NPT spout outlet.
2) What connection does the TOTO TS100EV wall spout use, threaded or slip-on?
It is a threaded spout with a 1/2 inch NPT connection. It will not work on a 1/2 inch copper slip-fit nipple, which needs a slip-on spout with a set screw. If your rough-in is copper stub-out, you need an adapter or a different spout.
3) Can I use this TOTO spout to switch water between the tub and the showerhead?
Yes. The TS100EV has an integrated lift-type diverter, so pulling the knob on top of the spout sends water up to the showerhead. It is intended for shower and tub applications. If you have a three-handle valve with its own diverter, you want a plain non-diverter spout instead.

Reach is 6 inches from the wall. That is enough to clear a standard tub roll rim on most alcove tubs, but on a tub with a wide deck or a thick tiled surround, measure from the finished wall face to the inside of the tub before committing.
