Concealed means the valve body mounts behind the finished wall, with only the sensor plate and spud connection exposed in the restroom. That takes a chase or a furred wall deep enough to house the valve and leave service access. Exposed-mount valves hang on the room side of the wall and need no chase, so if you're retrofitting a solid wall with no cavity, an exposed flushometer is the easier path.
