1) What does fail safe mean on the Accentra 4780LN electrified lever lock, and what happens in a power outage?
Fail safe means the outside lever is unlocked when power is removed. The 4780LN is an electrified cylindrical lock, so cutting or losing the 12VDC or 24VDC supply leaves the opening free for entry from both sides. That is the configuration used on stair and egress doors that must release on a fire alarm. If you need the door to stay secure when power drops, a fail safe lock is the wrong choice and you want a fail secure electrified lock instead.
2) Will the Yale/Accentra 4780LN Monroe lever fit a 1-3/8 inch door?
Yes. The 4700LN chassis is adjustable from 1-3/8 inch to 1-3/4 inch, so it covers both the standard 1-3/4 inch commercial door and older 1-3/8 inch interior doors. If your door runs 1-3/4 inch to 2 inch, that is the plain 4700 chassis range, not the LN.
3) What backset and latch throw does the 4780LN use, and will it drop into my existing strike?
Standard backset is 2-3/4 inches with a 1/2 inch stainless steel latchbolt throw, and it ships with the ANSI strike at 4-7/8 x 1-1/4 inches with 1-1/4 inch lip to center. If your frame is already prepped for that ANSI strike and a 2-3/4 inch backset bore, the 4780LN drops in. A T-strike is available to order on the 4700LN if your frame is cut for one.
4) Is the 4780LN fire rated for a labeled door?

12) I only need a non-electrified Grade 1 lever on the rest of the doors. What matches?

14) Is a Grade 2 lever such as the Hager 2580 or Cal-Royal AG05 an acceptable substitute for the 4780LN on a high traffic opening?
They are a different certification level and a different operating type. The Accentra 4780LN Monroe is ANSI/BHMA Grade 1 and electrified; the Hager 2580 Withnell storeroom lever and the Cal-Royal AG05 storeroom lever are both Grade 2 mechanical storeroom locks for 1-3/8 inch to 2 inch and 1-3/8 inch to 1-3/4 inch doors respectively.
