2) Is the ST-386 a lock body or just a cover plate?
Just the plate. The ST-386 is a blank conversion plate, so it fills and covers an abandoned hardware prep, it does not latch, lock, or accept a cylinder. If the old lock is coming out and nothing is going back in that spot, this is the part. If you are replacing one lock with another that has a different prep, you need the new lock plus this to close what the new one does not cover.
3) Is the Don-Jo ST-386 conversion plate paintable?
