These Crouse-Hinds covers are built for 4 inch square steel boxes, measuring 4 in by 4 in. That is the standard 4-square (1900 box) footprint used in commercial rough-in, so the cover lines up on the box's mounting ears without an adapter. All eight versions, blank through raised 1-1/4 inch, share the same 4 inch square base.
2) Which Crouse-Hinds 4-inch square cover do I order for a device versus just closing off a junction box?
Two different jobs. For a junction box you are only closing up, take the TP472 flat blank, or the TP478 flat blank with a 1/2 inch knockout if a fixture stem or fixture wire has to pass through. For a device, take one of the open covers with 2-2/4 inch ears: TP474 flat, TP476 raised 1/2 inch, TP477 raised 5/8 inch, TP475 raised 3/4 inch, TP479 raised 1 inch, or TP483 raised 1-1/4 inch.
3) How do I pick the raise height on a 4-inch square device cover?
Match the raise to the wall finish over the box. The raised versions come in 1/2 inch (TP476), 5/8 inch (TP477), 3/4 inch (TP475), 1 inch (TP479), and 1-1/4 inch (TP483). Where the box is set behind drywall or tile, the raise carries the device out to the finished surface so the plate sits flat; 1/2 inch matches common drywall and 5/8 inch matches the heavier board used in rated assemblies. If the box is flush with a finished surface already, the flat TP474 open cover is the one you want.
4) Are Crouse-Hinds 4-inch square steel box covers rated for hazardous locations?
No. These steel 4-inch square covers are for ordinary, non-hazardous locations only. If the opening is in a classified area (Class I, II or III), a cover from this group is the wrong part and you need a device or box listed for that class and division instead.
5) Are these Crouse-Hinds box covers UL listed and NEC compliant for an inspected commercial job?
Yes. The 4 inch square steel covers are UL listed and built to NEC/CEC standards, so they satisfy both US and Canadian code requirements on an inspected install. That is what an AHJ looks for on a junction box closure.
6) What does 'open with ears' mean on a TP474 or TP479 cover?
The ears are the two mounting tabs on either side of the 2-2/4 inch device opening.
