1) Does the Emtek 1573 multi point trim include the multi point lock body, or is it handles only?
Trim only. The Emtek 1573 set includes the plates and handles, and the multi point mortise lock box is not included. You order the lock body separately, matched to your door manufacturer's prep. If you already have a working multi point lock in a patio door and just want new handles and plates, this is the right kind of part; if the lock itself is broken, trim won't fix it.
2) How do I know if Door Configuration #5 is the right Emtek multi point trim for my patio door?
Configuration #5 on the Emtek 1573 means a Euro profile cylinder sitting below the handle, with a non-keyed American style thumbturn on the inside. Pull the existing interior escutcheon and look at where the cylinder hole falls relative to the lever: below the lever, with a thumbturn inside rather than a second keyway, is config #5. The plate is drilled for that layout, so a lock prepped with the cylinder above the handle or with a keyed interior will not line up.
3) What center-to-center spacing does the Emtek Concord multi point trim use?
3-5/8 inches from the center of the handle spindle to the center of the cylinder hole. That measurement is the one that decides fit. Measure your existing plate the same way before ordering, because a multi point lock prepped at any other spacing will leave the cylinder off the hole even though the plate itself covers the door.

Euro profile. The plate is cut for a Euro cylinder positioned below the handle, which is what multi point locks on swinging patio doors use. A standard American mortise cylinder will not fit the opening, so buy a Euro profile cylinder in a length matched to your door thickness and lock body. The interior side takes no cylinder at all on this one, it uses a thumbturn.
6) Should I get keyed multi point trim or the thumbturn version for a back patio door?
The 1573 is the non-keyed configuration: keyed cylinder outside, thumbturn inside. That is the normal choice for a residential patio or back door, where you want to throw the multi point from inside without hunting for a key. Choose keyed-both-sides trim instead only where code or insurance requires the inside to be key operated, or where there is glass beside the lock and you do not want a turn reachable through it.
