Two versions of the 4 inch ultra shallow IC air tight housing are sold: new construction, MPN H245ICAT, and remodel, MPN H245RICAT. Remodel is what you order when the ceiling is already finished and the housing has to go in through a cut hole from below. Same 4 inch aperture and same 15 W LED rating either way, so the choice is purely about how you get it into the ceiling.
