The C10-12 is sized for 12-10 AWG conductors. Stranded copper in that range crimps into the barrel correctly; 14 AWG and smaller will not hold properly in it, and anything above 10 AWG won't enter the barrel.
2) Is this ring terminal insulated, and do I need to sleeve it?
It is a non-insulated terminal, bare metallic copper with no nylon or vinyl sleeve. If the connection needs insulation for your application, plan on heat shrink or tape over the barrel after crimping.
3) Is the C10-12 ring terminal UL listed for inspected commercial work?
