An intermediate pivot goes in when frame conditions won't allow a standard top pivot installation. The M190 is a full mortise load-bearing pivot that carries lateral load at a point between the top and bottom of the door, keeping the leaf in alignment. It also helps during install of the floor closer and bottom pivot by holding the door where it belongs while you set the other hardware. It isn't a replacement for the bottom pivot or the closer, it works alongside them.
2) Does the M190 intermediate pivot come in a 1-1/2 inch offset as well as 3/4 inch?
Both. The standard configuration is a 3/4 inch (19mm) offset, and a 1-1/2 inch (38mm) offset version is available. The offset has to match the offset of your top and bottom pivots and the floor closer, so order the intermediate to suit the pivot set already on the opening, not the other way round.
3) Is the Norton Rixson M190 pivot handed, and how do I order the right hand?
Handing matters here. The M190 is a handed pivot and is ordered Left Hand or Right Hand. Determine handing from the door as you stand on the side it swings away from, and order to match. A left hand unit will not serve a right hand leaf.
4) Can I use the standard M190 pivot on a fire rated door?
No. The standard M190 is built with non-ferrous base metal and is not fire rated. For a fire door assembly you need the ferrous-material version, specified as FM190, which includes a 20-minute label option. That labeled option is orderable as the 3/4 inch offset with 20 minute label. Don't substitute the non-ferrous unit into a rated opening.

Match to the existing pivots and closer cover plate on the opening. Most of the mortised body is hidden once the door is hung, so only the exposed leaf shows.
