1) Can I widen an existing threshold with the Pemko EXT extender, or do I have to pull the whole sill?
The sill stays. A latching component at the lower end of the extender's slope hooks onto the threshold extrusion, so the existing Pemko sill is not removed and the joint itself needs no fasteners. The requirement is the extrusion width: it has to be a 4-7/8 inch or 5-3/4 inch Pemko extrusion for the hook to engage. This is the usual fix on a pre-hung opening where the jamb turned out deeper than the threshold that shipped with it, and on renovation work where a new floor or a thicker jamb left the sill short of the exterior edge.
2) Will a Pemko EXT sill extender hook onto another brand of threshold?

4) Do Pemko EXT threshold extenders come with screws and pre-drilled holes?
Neither. The EXT extenders are furnished un-drilled with no screws or nails supplied, so plan on drilling and providing your own fasteners for whatever the sill sits on, concrete, wood subfloor, or a mudsill.
