1) Does the Ocal LB47-G come with a cover, or do I order that separately?
The cover ships with it. The LB47-G includes a matching PVC-coated cover, so the body arrives complete as an assembly. You do not need to order a separate cover or gasket kit to close it out.
2) Will a PVC-coated LB conduit body hold up in a wastewater treatment plant?

An LB turns the run 90 degrees with the opening on the back, which is what you want coming out of a wall and down (or up) a conduit run while keeping a pull point. LL and LR bodies turn 90 degrees to the left or right instead, and a T body gives you a third entry for a branch. The LB47-G is the back-access 90 degree version, so pick it when the direction change is front-to-side and you want the cover facing you for pulling.
5) Can I use the LB47-G as a pull point for conductors, or is it just a bend?
Both. The removable cover makes the LB a legitimate access and pulling point along the run, so electricians use it at direction changes to get a hand on the conductors instead of fighting a full 90 degree sweep. Conductors enter and exit at 90 degrees.
