1) Is the Bobrick B-275 surface mounted to the wall or does it sit on the floor?
It mounts to the wall surface, not the floor. That keeps the floor clear under the unit for mopping and auto-scrubbers, which is why it gets specified in schools and high-traffic public restrooms. Wall backing or blocking needs to be adequate for a loaded 20-gallon receptacle.
2) Does the Bobrick B-275 come with a liner, or do I need to order bags separately?
A reusable vinyl liner is furnished with the unit. Hooks inside the receptacle hold it in place, and the liner lifts out for servicing. So you are not buying disposable bags to make it work, though many facilities still drop a poly bag inside the vinyl liner for wet waste.
