The Carlon blue square PVC covers are made for non-metallic boxes, and the A400R-CAR is the blank 4-square version. If your box is a metal 4-square, this is the wrong family; these are matched to Carlon's non-metallic boxes.
2) What is the difference between the 1/2-inch and 5/8-inch raised Carlon PVC box covers?
The raise is how far the device seat is brought forward out of the box. Carlon offers both depths in each gang count: A410R-CAR is the 1-gang 1/2-inch raised, A411RR the 1-gang 5/8-inch raised, A420RR the 2-gang 1/2-inch, and A421RR the 2-gang 5/8-inch. Pick the raise that brings the device flush with your finished wall surface, so thicker drywall or a furred-out wall usually takes the 5/8-inch.
3) I need a blank cover to close off an abandoned outlet box - which Carlon PVC cover do I order?
