1) What lock does the Don-Jo 17-CW wrap around plate fit?
It is cut for Schlage Cobra locks, with the electronic lock hole prep already punched and a latch prep indent formed in for the latch. That means it goes on without field cutting for that lock. It is not a universal wrap: put it over a different lock body and the prep will not match.
2) Can a wrap around plate cover up damage or an old cutout around a lock on a commercial door?
That is exactly what a wrap around is for. The 17-CW is a steel skin that folds over the door edge and covers the face around the lock area, hiding chewed-up or splintered material and reinforcing the door where the lock is mounted. It covers a 5-1/8 by 15 inch area on the face. What it will not do is fill a hole in the wrong place: the plate is prepped for a Schlage Cobra lock at a 2-3/4 inch backset, so the existing bore has to fall where the plate is punched.
3) Do I need a wrap around plate or just a filler plate over the old lock hole?
Different jobs. A filler plate is a flat plate that blanks off a hole you are no longer using. A wrap around like the 17-CW keeps the lock in service: it wraps the door edge, adds steel around the lock area, and carries the prep for the lock to pass through, in this case a Schlage Cobra with the electronic lock hole and latch indent already formed. If the lock is staying and the door face around it is worn or weak, you want the wrap. If the opening is being blanked off, you want a filler.

Four: 10B oil rubbed bronze, BZ satin bronze, PB polished brass, and S satin stainless. On a fleet of doors, satin stainless is the one that reads closest to the 630 lever trim you will usually find already on commercial openings; oil rubbed bronze and polished brass are for matching existing decorative hardware rather than blending into it.
6) Will one 17-CW cover both sides of the door?
A wrap around wraps the edge and covers one face, and the 17-CW ships one to a box. On a door that is damaged on both faces, or where you want matching trim inside and out, plan on two per opening.
