1) What conduit size do I order for the Crouse-Hinds 496 conduit strap?
The 496 two-hole strap is ordered by trade size, and it runs 1/2 inch (496-3) through 4 inch (496-12). The full ladder is 1/2, 3/4, 1, 1-1/4, 1-1/2, 2, 2-1/2, 3, 3-1/2 and 4 inch. Match the strap to the trade size of the conduit you are running, not to the measured outside diameter, since rigid and IMC are called out by trade size.
2) Will a Crouse-Hinds 496 strap work on EMT, or is it only for rigid and IMC?
It is a rigid/IMC strap. EMT of the same trade size has a smaller outside diameter than rigid or IMC, so a 496 strap will sit loose on EMT and will not clamp the run properly. For EMT, order an EMT-specific two-hole strap in the matching trade size instead.
3) Two-hole strap versus one-hole strap for rigid conduit, which should I use?
The 496 is a two-hole strap: it takes a fastener on each side of the conduit, so the run is pinned down at two points and cannot rotate or rock in the saddle. That is what you want on larger rigid and IMC, on horizontal runs carrying real weight, and anywhere vibration is a factor. A one-hole strap goes faster and is fine on small conduit tight to a surface, but it lets the pipe pivot around the single screw. On 2 inch and up, most installers stay with two-hole.
4) Are the mounting screws or anchors included with the Crouse-Hinds 496 conduit strap?

Rigid and IMC are secured within 3 feet of each box or fitting, then supported at intervals along the run, with the allowable interval widening as trade size increases. Count your boxes and fittings, add the intermediate points, and add spares, since the 496 is sold per size and a short pick on the last support stops the job. Mixed-size racks need a strap per size: a 2 inch strap will not hold 1-1/2 inch conduit tight.
7) Can I stack or gang Crouse-Hinds 496 straps on strut for a multi-conduit rack?
Yes. The two-hole pattern lets the strap bolt down flat to strut, to a channel, or to a backing plate, with each conduit carried in its own strap.
