1) How does the Kaba Ilco 4015 threshold bolt lock a pair of doors without a mullion?
The THB-4015 is a two-point lock for double door openings. It mounts to the rear of a pivoting deadbolt, so when you throw the deadbolt the bolt drops down into the threshold at the same time. That gives you a bolt into the frame and a bolt into the floor off one key throw. Add a header bolt to the same deadbolt and you get three-point locking, top and bottom plus the deadbolt itself.
2) Will an 11/16 inch bolt throw be enough to engage the floor strike on a storefront threshold?
The 4015 threshold bolt throws 11/16 inch (17.45 mm) into the threshold. That is a short drop, so the strike hole has to sit directly under the bolt in the threshold itself, not in a recessed floor below it. If your existing bottom strike is deeper than that, or the door has a large undercut over a finished floor, the bolt will not pick it up.
3) Can I add a threshold bolt to double doors that are already installed, or does it have to be ordered with the deadbolt?
It retrofits. The 4015 threshold bolt attaches to the rear of an existing pivoting deadbolt, so you can add it to a door already hung and keyed rather than replacing the lock. You are drilling for the bolt in the threshold and running the rod, not re-prepping the stile.

The rod runs from the deadbolt down to the threshold at a 34 inch (863.6 mm) cylinder height. That is the height the bolt is built around, so it suits a standard storefront door with the deadbolt at typical lock height. On a door where the deadbolt sits materially higher or lower than 34 inches off the threshold, the geometry will not line up.
5) Should I use a threshold bolt or a header bolt on my double doors?
They do opposite ends of the same job. The threshold bolt drives down into the threshold; the header bolt drives up into the head. Either one on its own, paired with the deadbolt, gives you two-point locking on the inactive leaf. Run both off the same pivoting deadbolt and you have three-point locking, head and floor and frame. If the opening only needs one, pick the end where the door has the most play.
6) Do I still need a flush bolt if I install a threshold bolt?
No, the threshold bolt does that job off the deadbolt instead. A conventional flush bolt is thrown by hand at the edge of the inactive leaf; the 4015 bolts into the threshold automatically when the pivoting deadbolt is actuated, so there is no separate lever to remember. What it does not do is secure the head, and for that you would add a header bolt to the same deadbolt.
