1) Can the Leviton 5226 combination switch and receptacle be used on a 277V lighting circuit?
The switch half is rated 15A at 120/277V AC, so the switch can control a 277V lighting load. The receptacle half is a NEMA 5-15R, which is a 125V straight-blade outlet and cannot be fed at 277V. If you run the switch at 277V you need to feed the receptacle from a separate 120V source, and the internal tab between the two halves has to be broken so they are on independent feeds.
2) What does the neon pilot light on the Leviton 5226 indicate, and does it need a neutral?
The 120V AC neon pilot glows when the switch is on, so you can tell from the wall that a remote load, an attic exhaust fan or an exterior fixture for example, is energized. A lit pilot lamp draws current on its own, so the device needs a neutral in the box. Plan on pulling a neutral to the box if the existing switch leg is a two-wire run with no white conductor.
3) Is the switch on this Leviton combination device single pole or three way?
Single pole, one pole only. The 5226 gives on/off control from one location. If you need to control the same light from two places, this device won't do it and you need a three-way switch at each end instead.
4) Do I need a single-gang or two-gang box for the Leviton 5226 switch/receptacle combo?
One single-gang opening. The switch and the duplex 5-15R outlet share a single device yoke, which is the point of the part: two functions where a two-gang box would otherwise be required. It takes a standard single-gang cover plate cut for a combination device, not a plain toggle or duplex plate.

The pilot light follows the switch either way.
6) Will a 15A combination switch/receptacle work on a 20A branch circuit?
A 15-amp device like the 5226 is permitted on a 20-amp branch circuit where more than one receptacle is on that circuit, which is the ordinary case in a residential or commercial lighting and general-purpose run. What it will not do is serve as the single receptacle on a dedicated 20A circuit, or accept a 20A T-slot plug. For a 20A appliance receptacle you need a 5-20R device instead.
7) Is this Leviton combo device a good fit for a bathroom exhaust fan with an outlet at the same spot?
That's one of the jobs it's built for. You get single-pole control of the fan, a pilot light so nobody leaves an attic or inline fan running unnoticed, and a 5-15R outlet at the same box. Note the outlet half is a standard duplex, not GFCI, so in any location where code requires GFCI protection you'll need to protect it from an upstream GFCI breaker or receptacle.
