1) Is the Milbank U3328-RXL meter socket the right one for a 3-phase 4-wire service?
The U3328-RXL is a 7 terminal, 3 phase socket rated 200 continuous amps at 600 VAC, 60 Hz, with a single meter position. Seven jaws is the standard count for 3-phase 4-wire network metering. If your utility calls for a 6 jaw or 5 jaw socket, this is not the one to order, since the jaw count has to match the meter form the utility sets.
2) Does this Milbank 200A socket have a main breaker or bypass built in?
No on both counts. The U3328-RXL is a straight meter socket: no main breaker, no bypass provision, and no receptacles. Overcurrent protection has to come from a separate disconnect or panel downstream. If your utility requires a lever bypass for hot meter changes, you need a different socket.
3) Can this meter socket be mounted outdoors, and can I feed it from underground?
Type 3R rated and UL Listed, so it is built for outdoor wall mounting with protection against rain and ice. It surface mounts, and it accepts either overhead or underground service entry, so the same can works for a mast drop or a lateral coming up from below.

The voltage rating is 600 volts AC, so 480Y/277 and 208Y/120 three phase systems both fall within it at up to 200 continuous amps, 60 Hz. The meter form your utility assigns still has to be a 7 jaw form for the socket to accept it.
