They split three ways. U5240 is a single pedestal for direct bury. U5241 is a double pedestal for direct bury, and it comes with dual mains (two 100A or two 200A) so it feeds two lots off one unit. U5242 is a single pedestal for pad mount, where you are setting it on a concrete pad rather than burying the post. Pick the number by how many services you are feeding and how the unit is being set, then pick the main from the suffix.
2) Does the Milbank U5240 come with a main breaker or do I have to buy one?
Both ways are sold. U5240-O and U5242-O are main breaker provision only, meaning the space and mounting are there but you supply the breaker. If you want it factory equipped, order a suffix: -100S gives a 100A main, -200S or -200P gives a 200A main. On the double pedestal, U5241-O-100S carries dual 100A mains and U5241-O-200S or -200P carries dual 200A. Decide this before you order, because the provision units ship without the breaker.

It is how the meter socket and the main breaker are tied together inside the pedestal, and utilities specify one or the other. Series wired routes the service through the meter and then the breaker in sequence; parallel wired taps them off the same line landing. Milbank builds both: U5240-O-200S is the series wired 200A, U5240-O-200P the parallel wired 200A, and the same S and P split runs through the double pedestal U5241-O-200S and U5241-O-200P. Ask your serving utility which they accept before you order, because the two are not interchangeable in the field.
4) What size service conductors can I land in a Milbank U5240 meter pedestal?
