1) Is the NGP 9590 door shoe rated for 90 minute fire doors, and on wood or hollow metal?
Ninety minutes, on both. The 9590 is UL certified to ANSI/UL10B and UL10C and to CAN/ULC-S104, for use on 90-minute rated wood and hollow metal fire doors, and it complies with NFPA 80 and NFPA 252. The UL10C listing covers positive pressure applications, which is what the IBC calls for on new work.
2) What length do I order for a 36 inch door, and does the NGP 9590 come long enough for a 48 inch leaf?
Two lengths are made, 36 inches and 48 inches. Order the 36 inch for a standard 3-0 leaf and the 48 inch for a 4-0. If your door falls between, take the next length up and cut it down; end caps are furnished standard so the cut end still finishes properly.
3) Do I have to specify handing on the NGP 9590 door shoe?
No handing to call out. The 9590 is a non-handed assembly, so one part number covers left and right hand openings. Orientation still matters at install, though, since the intumescent shoe goes on the push side of the leaf and the neoprene sweep on the pull side. For a stock of fire doors across a building that means you carry one item, not four.

The NGP 9590 does both: the neoprene sweep on the pull side restricts smoke passage under normal conditions, and the intumescent element in the stainless shoe on the push side expands under fire to seal the undercut. That combination is why it carries the UL10B, UL10C and CAN/ULC-S104 certification for 90-minute doors, and why a standard sweep is not a substitute when your undercut exceeds what NFPA 80 permits.
