1) Which NSi Polaris IT connector do I order for tapping a 4 AWG branch off a 250 MCM feeder?
Pick the connector whose range covers both the run and the tap. The Polaris 2-port IT line is sold by wire range: IT-4 (4 to 14 AWG), IT-1/0 (1/0 to 6 AWG), IT-3/0 (3/0 to 6 AWG), IT-250 (250 MCM to 6 AWG), IT-500 (500 MCM to 4 AWG) and IT-750 (750 MCM to 1/0 AWG). For a 250 MCM feeder with a 4 AWG tap, IT-250 covers both conductors. Note that IT-750 bottoms out at 1/0, so it will not accept a 4 AWG tap.
2) Will a Polaris IT connector accept fine-stranded or flexible cable?
The IT series is listed for Class B and Class C stranded conductors. That covers standard building wire stranding and the finer Class C. Welding cable, DLO and other Class G/H/K fine-stranded conductors are outside what this connector is listed for.
3) Do I still need to tape or heat-shrink a Polaris insulated tap connector?
No. The black Plastisol housing is the insulation, so the connection is finished once the conductors are torqued and the ports are fully seated. That's the point of the insulated tap over a bare split-bolt or mechanical lug, which you would have to tape out by hand.
4) How many conductors does a Polaris 2-port insulated tap connector take?
Two. The 2-port body lands one conductor per port, so it handles a single run-to-tap or splice pair. If the job needs a third or fourth conductor into one connection point, a 2-port body won't do it.
5) What's the largest conductor the NSi Polaris 2-port IT connectors handle?
750 MCM, using the IT-750, which ranges down to 1/0 AWG. Below that the IT-500 takes 500 MCM down to 4 AWG, and at the small end the IT-4 goes from 4 down to 14 AWG. Between them the range spans 750 MCM to 14 AWG.
6) Is a Polaris insulated tap a code-legal replacement for a split-bolt splice in a junction box?
For a two-conductor tap or splice it is a direct alternative: cULus listed at 600V and 90 degrees C, with the insulation built into the Plastisol body rather than applied as tape. The practical limit is conductor count, since the 2-port version lands exactly two conductors. Multi-port Polaris bodies exist for connections with more than two wires.
