1) Does the Oatey 43513 closet flange fit 3 inch or 4 inch PVC pipe?
Both, but in two different ways. The Oatey 43513 solvent welds inside 4 inch PVC Sch. 40 DWV pipe, or over the outside of 3 inch PVC Sch. 40 DWV pipe. So a 4 inch stack takes the hub inside it, and a 3 inch stub slips up into the flange. Either way it is a solvent weld joint, so you need PVC primer and cement, which are not part of the flange.
2) Can I rotate the flange ring after gluing it to get the closet bolts lined up with the toilet?
Yes. The metal ring on the 43513 rotates on the PVC hub, so you can set the bolts square to the wall after the joint is welded. There are six screw holes in the ring, which gives you options when a joist or a bad spot in the subfloor sits under one of them.
3) Is this Oatey closet flange code approved for a commercial or multi-family job?
The 43513 is UPC listed and meets ASTM D2661. Those are the listings most inspectors want to see on a DWV closet flange. Material is PVC with a white finish and a metal mounting ring for added strength at the screw holes.
4) Is a test cap included with the Oatey 43513?

No. The 43513 uses Oatey's patented bolt slot shelf, which holds the closet bolt head captive in the slot instead of letting it drop through. That is the difference between setting a bowl single-handed and fishing a bolt out from under it.
6) Does the Oatey 43513 sit flush on the finished floor or does it need to be raised?
It is a flush-mount flange. The 43513 is made to sit flat on top of the finished floor, which is where a flange should land so the wax ring compresses correctly. If your pipe is cut too low and the flange would end up below the finished floor, you need a spacer or extension ring rather than stacking wax.
7) Should I use a metal ring flange or an all-plastic one for a high-traffic restroom?
Go metal ring. The steel ring on the 43513 carries the closet bolt load and the floor screws far better than a plain PVC ring, which is the part that cracks when a bowl rocks or gets over-tightened. For apartment turns and commercial restrooms where the toilet gets reset repeatedly, the reinforced ring is the one that holds up. All-plastic flanges are fine for a one-off residential set.
8) Is the Oatey 43513 an offset flange?
No, it is a straight flange. The outlet is centered under the ring, so it will not shift the toilet's rough-in. If your waste line sits off the 12 inch rough-in point you need an offset closet flange instead, not this one.
