1) Can I still line up the toilet bolts if the flange cements in slightly off square?
Yes. The mounting slots are radial and extend 130 degrees, so the closet bolts slide along the arc and let you square the water closet to the wall even when the flange itself ended up rotated a few degrees off. That is the main reason to pick the long-slot version over a fixed-hole flange.
2) Does the Oatey 43511 come with a test cap for the rough-in pressure test?
No. The 43511 is the version without a test cap, so if your inspector wants the DWV system held for a water or air test, you need to plug the line another way (a separate test plug or a knock-out cap flange). If you want the cap built in, order the test-cap version of the flange instead.
3) Is the ring on the Oatey 43511 metal or plastic, and does it matter in a wet floor?
