That's exactly what the FS full surface hinge is built around. It mounts to the exposed faces of the door and frame, so it doesn't need the special hinge reinforcements a mortised continuous hinge or butt hinge prep requires, and it works with standard hollow metal door and frame manufacturer specs. Fasteners come in the box: 12-24 x 11/16 inch pan head self-drilling screws for the frame and 12-24 x 7/16 inch FHUC threadforming screws for the door. Wood screws are available on request if the door is wood.
2) Can I use this Pemko continuous hinge on a 3-hour fire rated door?
Yes, up to a 3-hour A-Label under UL10C, on approved hollow metal and wood door assemblies in drywall or masonry construction. One condition decides it: firepins are required for 3-hour fire rated doors, so they have to be part of the order and the install for that rating to hold. Assembly size limits are 4 ft 0 in x 10 ft 0 in for a single and 8 ft 0 in x 10 ft 0 in for a pair.
3) Is a full surface continuous hinge or a concealed continuous hinge the right choice for a retrofit?
Full surface is the retrofit answer. Because it screws to the outside faces of the door and frame, you don't have to machine the door edge or add frame reinforcement, which is what makes it workable on an opening that's already hung and already failing on worn butt hinges. Concealed and half surface types mount into the hinge edge and generally need prep or reinforcement that a retrofit opening doesn't have. The trade-off is appearance: a full surface hinge is visible on the face of the door, projecting 15/16 inch, so on an opening where the hardware has to disappear a concealed type is the better call.

5) How much does a Pemko full surface continuous hinge stick out from the door face?
The hinge measures 15/16 inch in height, and being a full surface type it sits on the door and frame faces rather than in the hinge edge. That matters where a door swings back to a wall or an adjacent frame, or where a wall-mounted stop or an overlapping surface closer arm is already in place. Check the swing clearance on the hinge side before committing to full surface on a tight opening.
6) Does a continuous geared hinge fix a commercial door that is sagging and dragging on the threshold?
That's the usual reason people move to one.
