1) Does the Thomas & Betts E971FB round floor box have knockouts, or do I have to use the hubs?
There are no knockouts on the E971FB. Conduit enters through the molded hubs only, so all raceway has to land on the hub entries. Plan your stub-ups around the hub positions before the pour, because you cannot open a side wall in the field the way you would on a knockout-style box.
2) Does the E971FB floor box come with a cover and receptacle?
The E971FB is sold as the box alone, one per unit. Covers, carpet or tile flanges, gaskets, and the receptacle are separate items. Match the cover to the finished floor you are working into, and order it on the same job so you are not holding a poured-in box open at trim-out.
3) Is a PVC floor box OK to pour in concrete, or should I use a metal one?
PVC is the usual choice for a concrete pour. The E971FB is molded PVC, which does not corrode from slab moisture or concrete alkalinity the way a steel box needs coating or galvanizing to resist. It mates naturally with PVC conduit systems. Where you need a box that will take heavy floor loading in an industrial setting, or where an equipment ground has to be carried by the raceway itself, a cast metal floor box is the alternative to look at.
4) Is 90 cubic inches enough for a floor box feeding a duplex receptacle and a through circuit?
90 cubic inches is a large volume for a single-device floor box, and it gives you room for a through-feed plus a device. Box fill still has to be calculated per NEC Article 314.16 using your conductor count, size, device, and grounds. Count the conductors you actually intend to land, not the ones you hope to.
5) Will a round floor box match the square floor boxes already installed elsewhere in my building?
It will not. The E971FB is a round body, and round and rectangular floor boxes take different cover plates and different carpet flanges, so trim will not interchange between them. If you are standardizing finished-floor openings across a property, keep to one body shape or accept two cover inventories.
