1) What is the difference between the Trimco 1098RGHA and the 1099RGHA anti-vandal pull?
Astragal. The 1098RGHA comes with an astragal, the 1099RGHA is the same pull without one. Both are the HA version, meaning they can be supplied with or without a cylinder prep and are mated to a specified mortise lock or deadlock. Pick the 1098RGHA when you need the astragal to cover the gap on a pair of doors or to shield the latch edge.
2) Can the Trimco 1098RGHA pull be used with an exit device or a cylindrical lock, or only a mortise lock?
The 1090 Series pulls can be mated to exit devices, mortise locks or cylindrical locks, and they are made for both in-swing and out-swing doors. The RGHA version specifically is built to mate to a specified mortise lock or deadlock. If you are working with a BEST or PDQ SFIC cylindrical lock, the 1098RG is the matching version, and for a PDQ LFIC cylindrical lock it is the 1098RGL.
3) Do I have to order the Trimco 1098RGHA with a cylinder hole, or can I get it blank?
Both are offered. The RGHA designation covers the pull with or without cylinder prep, so you specify which you need when ordering. Order it prepped when an exterior cylinder passes through the pull, blank when the pull is purely a grip and the locking is handled elsewhere on the door.
4) Will an anti-vandal pull actually stop someone attacking the exterior cylinder?

Standard construction is 1/8 inch type 316 stainless steel, with bronze and brass also offered. Trimco publishes a template for it, T19593, which covers both the 1098RGHA and the 1099RGHA. Use that template to mark the mounting and cylinder locations before you cut, and coordinate it with the prep for the mortise lock or deadlock the pull is being mated to, since the pull location is driven by the lock.
