1) What wire size does the Blackburn ATL302 compression lug take?
The ATL302 is sized for 3/0 conductor. It's an aluminum compression lug, so the barrel is crimped onto the cable rather than torqued down on a setscrew, and being aluminum bodied it's rated for both aluminum and copper conductors of that size.
2) Is the ATL302 lug UL listed, and what voltage is it good for?

Yes. Aluminum bodied compression lugs of this type are dual rated for aluminum and copper conductors, which is why a single part number covers both on a 3/0 termination. Use an oxide inhibitor on aluminum conductor and crimp with the ruby die called out on the barrel.
5) Do I need a special tool for the Blackburn ATL302, or can I bolt it on?
This is a compression terminal, not a mechanical setscrew lug. It has to be crimped with a hydraulic or battery compression tool using the ruby die. If you want a termination you can install with a hex key and a torque wrench, you need a mechanical lug instead, not this one.
