1) Is the Ultima Selectronic urinal valve hardwired or does it run on a battery?
Battery powered. A CR-P2 lithium battery rated for 8 years comes installed in the valve, so there's no transformer, no conduit, and no electrician needed for a retrofit. That's the usual reason property managers pick this over a hardwired sensor valve in an occupied building.
2) What happens if the battery dies or the water shuts off on this touchless urinal valve?
Two things cover you. The valve has fail-safe operation, meaning it closes automatically if power or water pressure is lost, so it won't stick open and flood the room. And there's a fully mechanical manual override that flushes without any power at all, so maintenance can keep the fixture in service until the battery is changed.
3) Is 0.125 gpf enough to clear a standard urinal, or do I need a 0.5 gpf valve?

No. The 6063013 is a urinal flush valve at 0.125 gpf with an 11-1/2 inch rough-in. A water closet needs a closet flush valve with the higher flush volume and the longer rough-in that fits a toilet spud. Use the urinal version only on urinals with a 3/4 inch top spud.
