1) Will the Adams Rite 4591 paddle work with my Adams Rite MS+1890 deadlock?
Yes. The 4591 deadlatch paddle is built for the MS+1890 Deadlock/Deadlatch, and it also fits the 4300, 4500 and 4900 Series Deadlatches and the 2190/2290 Interconnected Deadbolt/Deadlatch. It is trim only, so the lock body has to already be one of those. 2) What do I have to specify when ordering an Adams Rite 4591 paddle?

Order the deadlatch or deadlock (4300, 4500, 4900, MS+1890, 2190/2290) as its own line, plus the mortise cylinder if the opening is keyed. The lock bodies in these families take a standard MS cam mortise cylinder from either side or both.
6) I am replacing a knob on an aluminum entrance door. Can I just swap the 4591 paddle onto the existing lock?
Only if the lock body is an Adams Rite 4300, 4500, 4900, MS+1890, or 2190/2290. The 4591 is made to drive those; it will not adapt to a cylindrical knob lock or a different manufacturer's narrow stile latch. If the door has a bored knob set, you are changing the lock too, not just the trim.
7) Is a paddle better than a lever for a door people go through with their hands full?
That is exactly what a paddle is for. Push or pull operation means a forearm, hip or cart edge opens the door, no grip and no twist.
