1) Do I need an expansion shield or sleeve with these 1/2 inch hex head screw anchors?
No. A large diameter screw anchor like the LCS1265 cuts its own thread in the concrete or masonry, so there is no shield, sleeve, or expansion wedge to insert first. You drill the hole, blow it clean, and drive the anchor with a socket on the hex head. That makes it faster to set than a wedge or sleeve anchor, and it can be removed and the hole reused in a way an expansion anchor cannot.
2) Is the zinc plating on the LCS1265 good enough for an outdoor or damp install?
Zinc plated carbon steel is a dry to occasionally damp interior finish. It handles a covered loading dock or a parking structure interior, but for anchors sitting in standing water, in coastal air, or in treated lumber contact, specify a stainless or hot dip galvanized anchor instead. The LCS1265 is carbon steel with zinc, not stainless.
3) What is a 6-1/2 inch long anchor used for, and is that length overkill for a conduit strap?
At 6-1/2 inches overall, this anchor is sized to pass through a thick standoff before it ever reaches the concrete, so it suits things like anchoring base plates through grout or a sleeper, heavy strut channel on a furred wall, or equipment feet with thick pads. For a one-hole conduit strap flat against the slab it is far longer than needed, and a shorter 1/2 inch screw anchor from the same large diameter family is the better buy.
4) Will a hex head anchor work where I need a flush finish?
It will not sit flush. The LCS1265 has a hex washer style head that stands proud of the fastened material so you can drive and later back it out with a socket. If the fixture needs a countersunk or flush surface, use a flat head or Phillips head screw anchor rather than this hex head version.
5) Can these be installed with an impact driver, or do I need a torque wrench?
An impact wrench with a socket on the hex head is the normal way to set a 1/2 inch large diameter screw anchor, but back off as the head seats. Over-driving strips the thread the anchor just cut in the concrete, and a spun-out hole means moving over and drilling again. Keeping the drilled hole clean of dust before driving matters just as much for holding power.
