1) Can I use a Crouse-Hinds Form 5 LR body in a hazardous or classified area?
No. The Form 5 LR malleable iron body is rated for ordinary, non-hazardous locations only. For Class I or Class II classified areas you need an explosionproof body from the appropriate Crouse-Hinds hazardous-location line, not this one. Don't substitute it into a classified space just because the threads match.
2) Is this threaded or set-screw? Will it take rigid conduit with cut threads?
Threaded hubs. The Form 5 LR is the threaded series, so it takes NPT-threaded rigid metal conduit or IMC, either factory threads or threads you cut and ream on site. It will not accept EMT directly, since EMT is not threaded. If you are running EMT you either transition with a threaded coupling/adapter at the body or use a set-screw style conduit body instead.
3) I am replacing an old LR condulet on a 3/4 inch rigid run. Can I thread this straight onto the existing conduit?
Yes, if the existing run is threaded rigid or IMC at 3/4 inch, order the LR75M and thread it onto the same conduit ends. Keep in mind the code side the fitting cannot do for you: a conduit body used as a pull point still has to meet conductor fill and bending-space rules for the trade size you pick, and it must stay accessible, not buried in finished construction.
8) Can I standardize on these conduit bodies across a healthcare or school facility?
That is a reasonable standardization play for ordinary, non-hazardous spaces. Stocking the malleable iron LR in the two or three trade sizes your runs actually use (commonly 1/2, 3/4 and 1 inch for branch circuits) keeps one letter, one material and one cover family on the shelf, so a maintenance electrician grabs the right part without a lookup.
