1) Does the Emtek 13A1 multi point trim include the multi point lock mechanism, or just the handles and plates?
Trim only. The 13A1 set gives you the plates, the handles, and the cylinder, and the multi point mortise lock box has to be sourced separately. If your patio door already has a working multi point gearbox in the stile and you are replacing worn or dated hardware, that is exactly what this is for. If the door has no lock body in it yet, you need the mortise lock box on top of this order.
2) What is the center to center spacing on the Emtek 13A1 trim plate?

Emtek sorts its multi point trim by door configuration, and the 13A1 is the #3 layout: a 1-1/2 by 11 inch plate with the cylinder hub above the handle, 4-1/8 inches center to center. Emtek references Andersen doors for this hardware. The configuration number is the fit decision, so match it to the prep in your door rather than picking on plate style.
4) Can I key this Emtek patio door trim to match my Schlage or Kwikset house keys?
Yes, and that is an ordering choice. The 13A1 comes with an American cylinder in either a Schlage keyway (the standard option) or a Weiser WR5 keyway, which is the Kwikset-family keyway. Pick the one your existing house keys run on, then choose keyed different or keyed alike, and if you have an existing bitting you can supply the key code at order time so the patio door runs on the same key as the rest of the openings.
