Both are the same 200 amp ringless, horn bypass pedestal. The U5138-O ships with a main breaker provision, meaning the space and mounting are there but you supply and install the breaker. The U5138-O-200S comes with a 200 amp main breaker already installed. Order the -200S if you want the service disconnect factory fitted; order the plain -O if the utility or your spec calls for a specific breaker.
2) Will the Milbank U5138 meter pedestal take 4/0 aluminum URD from the transformer?
Yes. The line side terminals accept 6 AWG through 350 kcmil, so 4/0 aluminum URD lands directly. Load side terminals take 1/0 AWG through 300 kcmil. Both are double mechanical lugs, and cable entry is underground, which is how mobile home park feeds are normally run.
3) Can this Milbank pedestal be mounted on a wall or a pole instead of a pad?
No. The U5138 is built as a pedestal unit, pad mounted on a single base, 56 inches tall and 9.688 inches wide. There is no wall or pole mounting provision on it. For a wall mounted service you need a standard surface mount meter socket, not this pedestal.
