1) Does the Nibco 4811-14-V PVC DWV test tee come with a plug?
No plug is included. The 4811-14-V is the without-plug version, so the threaded branch ships open and you supply the test plug or cleanout plug that suits the job. The branch is female NPT, so a standard threaded plug in the matching pipe size threads straight in.
2) How do the ends of this PVC test tee connect, glue or thread?
Both ends of the run are hubs for solvent cement, and the branch is female NPT thread. So you cement the tee into the DWV line and thread the plug or test apparatus into the side outlet. Mixed connection on one fitting: hub x hub x FIPT.
3) Can I pressure test a DWV line with air using this Nibco test tee?

6) Will a PVC test tee hold up in a commercial building drain, or should I be using cast iron?
PVC DWV will not rust, scale, pit or corrode, and Nibco lists this fitting for both commercial and residential plumbing service. The limit is heat: 140 degrees F maximum. For a commercial kitchen or a line seeing sustained higher-temperature discharge, PVC is the wrong material and a metallic DWV system is the answer.
7) Is this Nibco fitting white PVC or the black ABS style?
White PVC. The 4811-14-V is polyvinyl chloride, and PVC is joined with PVC solvent cement to PVC pipe. It is not ABS and should not be cemented to black ABS pipe with standard cement; that joint needs a transition coupling or an ABS fitting instead.
